New patient registration

To join the Practice please visit the practice with the completed forms and book a New Patient Check appointment. You will need the  registration form (GMS1W), and a health questionnaire.

You will receive a health check with the healthcare assistant and will be asked to complete a questionnaire on your personal history to assist in giving you treatment until your records are received from your previous doctor.

When all the above is undertaken you will be registered with the practice.

For all children under the age of 5 we will also require a copy of the child’s immunisation records. We also require immunisation records for all non UK patients under the age of 16.

Updating your contact details

Please let us know as soon as possible if you change your address, telephone number or name. It is important that we have up to date contact details in case we need to contact you. You can complete this on the form below.

Please note: If your name has changed due to Marriage or by Deed Poll, can you please provide us with a copy of the appropriate document.

New Patient Registration

DURING CORONAVIRUS (COVID-19) PERIOD

We welcome new patients.

We require all new patients to complete a registration form (GMS1W), and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. Your NHS number is also required to register, this can be obtained from your previous surgery. Please also supply a copy of any repeat medications from your previous GP to ensure we have proof of medication. 

Please complete and return to Ashgrove Medical Centre. 

For patient safety and confidentiality; please note by returning a completed form you are signifying you are who you say you are and all information is correct to the best of your knowledge. This will mean you also give permission for us to set you up as a New Permanent Patient at our Practice.
